Payment for Teletherapy services will need to be made via debit/credit only.
Insurance Information
I am currently credentialed with Anthem and Medicare Insurance Plans. I am a non-network approved provider for Tricare. For clients with all other insurances, I am considered an “out-of-network provider.” I will provide all the information needed to collect out-of-network benefits for any other insurance. Please note that as an out-of-network provider, I will not bill your insurance company directly or accept payment directly from them. It will be your responsibility to pay me at the end of each session and to submit the claim to your insurance company. Please contact your insurance company to understand all out-of-network compensation plans. Please note that all insurance companies require a diagnostic code to provide reimbursement.
Cancellation/No-Show Policy
Please note that I require that you provide at least 24 hours notice if you cannot keep a scheduled appointment, so that I may offer that appointment time to someone else. If you are unable to provide 24 hours notice, or if you miss a scheduled appointment, you will be charged a late cancellation fee or missed appointment fee respectively. Insurances do not cover late cancellation or missed appointment fees.
